DSPL 8 provides and supports a number of transition-related activities. 

1) Dacorum Schools Transition Tool has been created to identify and support schools with transition priorities. It is attached below and all year 6/7 transition conversations should be shaped by this tool. It can also be used for year 2/3 transition conversations. 

2) HfL has developed Smooth Transitions for early years and is available below. 

3) DSPL 8 funds Bounce forward to support parents with transition and also supports a local schools partnership parent evening in Summer term. 

4) DSPL 8 will seek to support and advise on learners who have EHCPs requiring high levels of support with transition to new setting, especially those who have been identified as awaiting special schools. Please contact DSPL manager to discuss these learners in the Spring term.
